Block 10 and 11

Block 10 is the first day worked for the employment period. Block 11 is the last day for which the employee was paid.

Block 12

The final pay-period end cannot be before Block 11 and must fit the selected pay-period type.

Example

an employee may stop mid-period, so Block 11 and Block 12 can differ.

Boundary or exception

Do not substitute the issue date, cheque date or expected recall date.
